Singapore Health Declaration for workers
Overview
The Singapore Health Declaration is a form that all travellers must fill out before entering Singapore. It is designed to help protect public health by providing information about the traveller’s health status and travel history. The form must be completed online and submitted at least three days before arrival in Singapore.
The Singapore Health Declaration can be found on Singapore eVisa. It requires personal information such as name, passport number, contact details, and travel history for the past 14 days.
Requirements
All travellers must complete the Singapore Health Declaration before entering the country. This includes workers who are travelling to Singapore for work purposes.
Travellers must also provide proof of a negative COVID-19 test taken within 72 hours prior to departure from their home country or last port of call.
Submission
Once the Singapore Health Declaration has been completed, it must be submitted at least three days before arrival in Singapore.
Travellers will receive an email confirmation once their declaration has been successfully submitted.
Penalties
Failure to submit the Singapore Health Declaration may result in penalties or refusal of entry into Singapore.
